Minnesota Collaborative Practice Agreement

For collaborative dental hygienists who provide services to participants in a managed care organization (MCO), contact the MCO for registration requirements and coverage. Individual dental hygienists in collaborative practice must register with a group membership to receive group-level reimbursement for all dental hygiene services provided to MHCP participants. As part of this process, the MHCP requires individual National Supplier Identifiers (NPIs) and membership in one or more of these registered group providers. Calculate all services provided under collaborative practice as follows: MHCP reimburses dental services covered as part of the collaborative practice dental hygienist practice, as specified in the Laws of Minnesota, 150A.10, subsection 1a.
